Continuous Care Centers offer a broad range of specialized services for adult patients. We realize that catastrophically ill patients require more than ordinary care.

Our team of suitably skilled professionals including nurses, internists, physical, occupational and respiratory therapists, dietitians, counselors, and social workers, are all dedicated to providing a high quality of continuum care.

Once the clinical goals are achieved, our patients may be discharged to their home, referred to an active rehabilitation program, or transferred to a skilled nursing facility or hospice.
