What is our mission statement?
The mission of Continuous Care Center is to provide high quality, affordable, compassionate, transitional health care to the people of Northeastern Oklahoma and surrounding communities.

What is the difference between LTACH's and Skilled Nursing facilities?
LTACH's are staffed and equipped to provide specialized and intensive treatment for critically ill patients, while Skilled Nursing facilities are generally structured to stabilize the condition of patients with much lower acuity levels. LTACH's also aggressively apply an interventionist treatment plan designed to secure the best possible outcomes.

How does a patient get admitted into your hospital?
Each patient referred to CCC receives an assessment by a nurse liaison. He or she performs a clinical assessment to determine the appropriateness for admission.

Can my current physician still take care of me?
A patient's current physician, as long as they have admitting privileges, can still take care of their patient, if this is what he/she desires to do. Of course, another attending physician can be assigned to care for the patient.

What about your financial criteria for patients?
The Admissions Administrator assists in this process of determining the financial resources for patients, available through Medicare, Commercial Insurance, etc., prior to admitting. The referring physician and or facility is then notified for admission.
