Patients are referred to Continuous Care Centers from a variety of sources. While many of our patients are referred from Acute Care Hospitals, we also receive recommendation from:

• Skilled Nursing Facilities
• Home Health Agencies
• Managed Care Organizations
• Nursing Homes / Assisted Living Centers
• Patients and Families

A smooth transition for our patients and families.

Our Nurse Liaison and Clinical Staff are always available to answer any questions. Tours of the facility and educational presentations may also be scheduled.
